Under the Care Act 2014, all carers have the right to an assessment which looks at their needs and the impact that their caring role is having on their wellbeing. It also gives carers the chance to talk about other things going on in their lives and look at what support they need to help them achieve the things they want to do day to day.
Trafford Carers Centre completes carers assessments for unpaid carers in Trafford, free of charge. An assessment can be completed over the telephone or in person with one of our workers (either at home or in another convenient place). Carers can be referred by filling out an online referral form, or by calling the helpline on 0161 848 2400.
What can carers expect when they contact or are referred to the Carers Centre?
We use the ‘3 Conversation Model’, designed to offer carers a person centred approach to support. Our caseworkers will talk to carers to best understand what is happening in their caring role and how it is impacting health and wellbeing. It allows for an immediate response to any needs and helps reach a more stable position before identifying long term support.
- Conversation 1 is designed to understand what really matters; connecting carers with resources and support that can help them continue with their caring role.
- Conversation 2 may happen if a carer finds themselves at breaking point or in a crisis. Quite often this is the point where someone feels that they can no longer carry on as a carer. A caseworker will work to identify what needs to change urgently to help regain control.
- Conversation 3 is about helping to build a good life for the long term. This will be a more intensive piece of work helping to identify what support is needed.
The Conversations are about us working together with carers to find the best support available to meet the needs of carers. There is no one size fits all.
